FACTORS WHICH CONTRIBUTED TO THIS ACCIDENT WERE: THE DUAL STUDENT'S IMPROPER USE OF THE FLIGHT CONTROLS, THE CFI'S FAILURE TO ISSUE PROCEDURES/DIRECTIVES TO THE STUDENT, AND THE CFI'S FAILURE TO MAINTAIN DIRECTIONAL CONTROL OF THE AIRPLANE.
