A DETERIORATED FUEL CAP, WATER CONTAMINATION OF THE FUEL, AND THE PILOT'S INADEQUATE PREFLIGHT OF THE AIRCRAFT. A FACTOR RELATED TO THE ACCIDENT WAS: THE DIRT BANK (BERM).
