On August 31, 2025, about 1720, mountain daylight time a Piper PA28-200R, N1147X, was substantially damaged when it was involved in an accident near Alta, Utah. The pilot was seriously injured, and the passenger sustained fatal injuries. The airplane was operated as a Title 14 Code of Federal Regulations Part 91 personal flight. Preliminary ADS-B data indicated that after departure from Bolinder Field/Tooele Valley Airport (TVY), the airplane initiated a climbing right turn to the northeast.
