FACTORS RELATED TO THE ACCIDENT WERE: DARKNESS, THE WEATHER CONDITIONS, AND FAILURE OF THE APPROACH CONTROLLER TO ISSUE AN IFR TRANSPONDER CODE TO THE AIRCRAFT WHEN HE PROVIDED THE IFR CLEARANCE, WHICH WOULD HAVE ALLOWED A LOW ALTITUDE ALERT TO BE TRIGGERED.
