Aircraft on the NTSB record: Schweizer 269C · serial S1593

A fatigue fracture of the main transmission drive pinion. Also causal was the manufacturer's failure to remove the affected pinions from service during the period that transpired between the initial failures and the accident flight.

ATL98LA065Accident on April 27, 1998 at San Juan, PR: no injuries, substantial damage.

Aircraft on the NTSB record: Schweizer 269C · serial S1593

The pilot's failure to adequately secure the cargo strap which became entangled in the tail rotor, leading to a loss of the tail rotor control.
