NTSB accident and incident record under N540AK

ANC22FA005Accident on November 7, 2021 at Kodiak, AK: fatal, substantial damage (1 fatal).

Aircraft on the NTSB record: Gerald Jones 2+2 · serial 001

The pilot's failure to maintain adequate airspeed while maneuvering, which resulted in an exceedance of the airplane’s critical angle of attack and an aerodynamic stall.

LAX01LA045Accident on November 19, 2000 at Guistine, CA: serious injuries, substantial damage.

Aircraft on the NTSB record: Collette GLASSAIR III · serial 3077

Maintenance personnel misrouted the crankcase breather hose so that it was pinched when the cowling was installed. This pressurized the crankcase displacing the oil seal and forcing oil out of the engine. A factor was the unsuitable terrain encountered during the forced landing.
