The NTSB's narrative final · quoted from the NTSB record
The propeller separated from the left engine after experiencing a propeller blade separation from its hub assembly. The pilot landed, gear-up, in an open area. A metallurgical examination on the separated propeller blade revealed fatigue cracking of its retention screw. The retention screw was the subject of two service bulletins which were issued by the manufacturer on October 23, 1981, and August 27, 1982. These bulletins required replacement of the older retention screws with the newer retention screws at the next overhaul. The recommended overhaul time for this propeller assembly is five calendar years or 2,000 hours, whichever comes first. The engine log book revealed the last recorded propeller overhaul was performed on January 5, 1978.

About this page
Everything above comes from the NTSB's public accident database: the narrative, probable cause and findings are the NTSB's own words, and the coded tables behind its report (pilot, aircraft, flight, weather, injuries, sequence of events) are written out in plain English. Pilots' ages, home towns and medical details are left out on purpose. The docket at the NTSB holds the report as a PDF and the investigation's photographs and documents. This site never names pilots, passengers or anyone else involved. A preliminary report describes what is known soon after the event and can change; the final report, with the probable cause, usually follows one to two years later and this page is refreshed when it does. Case number LAX93LA338.
